An HDMI audio extractor that supports all video and surround formats AND also has:

a simple dial on the side that allows me to adjust the audio delay coming out of the analog and optical outputs.

Maybe someone has that already. If so enlighten me.

A "simple dial..."? These circuits are fast, but it takes time for some processes to happen. The Hall Technologies analog delay that has been referred to here (NLA) is on the Markertek site with a price of $599- how much do you want to pay for this?

My mechanic told me, "I couldn't repair your brakes, so I made your horn louder."

This is the winning question. It's not that it isn't possible to build, but more so if the development and production costs would ever be recovered if it came to market. We'll definitely keep our eyes peeled to see what can be done and hope for an economical chipset to allow it to be priced realistically.

If its cost prohibitive I guess I will use this.

Sescom SES-A-V-SYNC A/V Lip-Sync Corrector-by-Sescom [Link: a.co]

It moves in steps so typically it works. The point I’m trying to make is an extractor is all well and good but it very often leaves us with a delay issue.

I’m trying to extract audio from an Xfinity box right now with the new Metra extractor and the analog audio isn’t even close to being in sync with the HDMI video.

I don't recall the 'box', but I encountered one situation where video was ahead of audio. No amount of audio delay can fix that.
