have you tried _0135 for the receiver? 0100? 0380?
DVD: 0503, or 0627?

Just for the heck of it, what device button are you trying to program the receiver on, and what device button for the dvd?
If you are trying to program the dvd to the "dvd" button, did you at some point change the dvd button to be some other device button?
If it wouldn't create a HUGE problem, you may want to do a mfr reset and try again. (If your other devices are using standard setup codes, jot them down so you can re-program them in.) You can find explicit instructions on how to do the mfr reset in this, but especially the OFA threads.
