What happens when it fails? Does it miss it by one each time or is it not changing anything? Figure out how to manually select the inputs in Device mode, then translate that to the Adjust Input settings.
Sometimes a device will get hosed - if all else fails delete the TV device completely and then re-add it. You could also try increasing the inter-device delay for the TV.
