Make sure the number and order of the inputs on the TV match the number and order of the inputs in the remote software and that the commands used to select inputs are correct. If it's consistently wrong the same way it's probably configured wrong. If it's random, then try increasing the inter-device delay for the TV. If you're using a cycling method, see if the TV device has discrete inputs that can be used instead.
