You don't need the middle LNB hooked up unless you want local channels from North Carolina. The Sat-C kit will come with a special LNB and connector to add it to your existing multiswitch.
Some people with more elaborate setups will use a standard LNB there alonge with some more switches to get both Dish and DirecTV service from one dish.
