For watching TV my cable box is used for changing channels (signal is component in directly to TV) and my audio is input to my receiver. So with a universal remote does the channel up/down key control the cable box and the volume up/down key control the receiver without having to change back and forth between device input?

Most universal remotes will have a procedure to allow you to control either the TV or the receiver volume for selected components.
If it is a learning remote, you can always learn the volume control commands from the receiver remote, in case the remote doesn't have the feature to allow you to use the receiver's volume.
